Recirculating aquaculture systems (RAS) recycle water by circulating it through filters to remove fish waste and food, and then recirculating it back into the tanks. This saves water, and the waste gathered can be used in compost or, in some cases, could even be treated and used on land. While RAS was developed with freshwater fish in mind, scientists associated with the Agricultural Research Service have found a way to rear saltwater fish using RAS in low-salinity waters. Although saltwater fish are raised in offshore cages or caught with nets in water that typically has a salinity of 35 parts per thousand (ppt), scientists were able to produce healthy pompano, a saltwater fish, in tanks with a salinity of only 5 ppt. Commercializing low-salinity RAS is predicted to have positive environmental and economic effects. Unwanted nutrients from the fish food would not be added to the ocean, and the risk of transmitting diseases between wild and farm-raised fish would be greatly reduced. The price of expensive saltwater fish, such as the pompano and cobia used in the experiments, would be reduced. However, before any of this can be done researchers must study every aspect of the fish's lifecycle, including the amount of ammonia and nitrate the fish will tolerate in the water, what to feed the fish during each stage of its lifecycle, the stocking rate that will produce the healthiest fish, etc. Some 16 countries now use geothermal energy for aquaculture, including China, Israel, and the United States.
== Chemistry == BmKAEP is an inhibitory β-toxin and thus, a Na+ channel inhibitor. As with other mammal and insect toxins, BmKAEP is classified according to species and mechanism of action. BmKAEP is a 61-amino-acid protein derived from an 85-amino-acid precursor. The mature protein contains 8 cysteine residues that establish 4 disulfide bridges (4C-C). Despite its high homology with other depressant toxins, BmKAEP differs from them at residues 6, 7 and 39, which is thought to be important in determining its unique function. Its lysine residue, at position 51, also has a special feature: it interacts with mammalian Na+ channels.

==== Lung cancer ==== A prospective clinical study (NCT05835570) involving 58 participants with non-small cell lung cancer (NSCLC) compared the diagnostic performance of 68Ga-Trivehexin PET/CT with 18F-FDG PET/CT. Both radiotracers showed similar diagnostic accuracy (100%, 58/58) for the detection of primary tumors. The sensitivity for detection of lymph node metastases was comparable for 68Ga-Trivehexin (80%) and 18F-FDG (72%), but 68Ga-Trivehexin showed a higher specificity (93.8%) and accuracy (91.2%) than 18F-FDG (62.5% and 64.2%, respectively). Sensitivity for detecting brain metastasis was 92.3% for 68Ga-Trivehexin and 38.5% for 18F-FDG, mainly because of the high glucose consumption of normal brain tissue, which usually results in a high physiological uptake of 18F-FDG in the brain, generating a strong background signal which frequently obscures brain metastases in PET images (see image). Application of 68Ga-Trivehexin PET was reported for single cases of other, rare forms of lung cancer, such as bronchial mucoepidermoid carcinoma and mucinous lung adenocarcinoma.

the revenue-generating potential of orphan drugs [was] as great as for non-orphan drugs, even though patient populations for rare diseases are significantly smaller. Moreover, we suggest that orphan drugs have greater profitability when considered in the full context of developmental drivers, including government financial incentives, smaller clinical trial sizes, shorter clinical trial times and higher rates of regulatory success. According to a 2014 report, the orphan drug market has become increasingly lucrative for a number of reasons. The cost of clinical trials for orphan drugs is substantially lower than for other diseases because trial sizes are naturally much smaller than for more diseases with larger numbers of patients. Small clinical trials and minimal competition place orphan agents at an advantage in regulatory review. Tax incentives reduce the cost of development. On average the cost per patient for orphan drugs is "six times that of non-orphan drugs, a clear indication of their pricing power". The 2014 Orphan Drug report stated that the percentage of orphan drug sales as part of all prescription drug sales had been increasing at a rapid rate. Vancomycin is recommended to be administered in a dilute solution slowly, over at least 60 min (maximum rate of 10 mg/min for doses >500 mg) due to the high incidence of pain and thrombophlebitis and to avoid an infusion reaction known as vancomycin flushing reaction. This phenomenon has been often clinically referred to as "red man syndrome". The reaction usually appears within 4 to 10 min after the commencement or soon after the completion of an infusion and is characterized by flushing and/or an erythematous rash that affects the face, neck, and upper torso, attributed to the release of histamine from mast cells. This reaction is caused by the interaction of vancomycin with MRGPRX2, a GPCR-mediating IgE-independent mast cell degranulation. Less frequently, hypotension and angioedema occur. Symptoms may be treated or prevented with antihistamines, including diphenhydramine, and are less likely to occur with slow infusion.
=== Mode of action === The condition for which SNRIs are mostly indicated, major depressive disorder, was thought to be mainly caused by decreased levels of serotonin and norepinephrine in the synaptic cleft, causing erratic signaling. Based on the monoamine hypothesis of depression, which asserts that decreased concentrations of monoamine neurotransmitters leads to depressive symptoms, the following relations were determined: "Norepinephrine may be related to alertness and energy as well as anxiety, attention, and interest in life; [lack of] serotonin to anxiety, obsessions, and compulsions; and dopamine to attention, motivation, pleasure, and reward, as well as interest in life." SNRIs work by inhibiting the reuptake of the neurotransmitters serotonin and norepinephrine. This results in increased extracellular concentrations of serotonin and norepinephrine and, consequently, an increase in neurotransmission. Most SNRIs including venlafaxine, desvenlafaxine, and duloxetine, are several fold more selective for serotonin over norepinephrine, while milnacipran is three times more selective for norepinephrine than serotonin. Elevation of norepinephrine levels is thought to be necessary for an antidepressant to be effective against neuropathic pain, a property shared with the older tricyclic antidepressants (TCAs), but not with the SSRIs. Recent studies have shown that depression may be linked to increased inflammatory response, thus attempts at finding an additional mechanism for SNRIs have been made.
Pembrolizumab, sold under the brand name Keytruda, is a humanized monoclonal antibody, more specifically a PD-1 inhibitor, used in cancer immunotherapy to treat many types of cancer. Common side effects include fatigue, musculoskeletal pain, decreased appetite, itchy skin, diarrhea, nausea, rash, fever, cough, difficulty breathing, constipation, pain, and abdominal pain. It is an IgG4 isotype antibody that blocks a protective mechanism of cancer cells, allowing the immune system to destroy them. It targets the programmed cell death protein 1 (PD-1) receptor of lymphocytes. Pembrolizumab was approved for medical use in the United States in 2014. It is on the World Health Organization's List of Essential Medicines. A fixed-dose combination of pembrolizumab and berahyaluronidase alfa, (Keytruda Qlex) was approved for medical use in the United States in September 2025.
